Please ensure you read this page carefully before you compile your entry.
Entering the Smile Awards couldn’t be easier! There are 15 categories for you to choose from. Each category has specific criteria to be submitted, please read these individually. Then simply send in your case report, include photos on a CD, form and payment to the address below. You may enter as many categories as you wish and don’t forget Patient’s Smile and Generosity of a Smile are free of charge!
Terms and Conditions
- Entries must be from the UK and Republic of Ireland or GDC registered
- Only one entry per category per person
- Please note the judges mark the entries anonymously. Please avoid using letter headed paper or your name on the actual entry.
- Entries will not be returned, please send copies.
- Photographs must be high resolution. As a guide, minimum number of 24 BACD (sequence). This does not apply to Patient's Smile entry.
- Please send in your supporting photos separately on a CD, not dropped into the word file.
- All entries must include an entry form and payment.
- The case report must be between 1,000 and 1,500 words detailing the treatment carried out. The level of detail should allow the judges to understand what treatment was carried out and why.
- The same patient must not be entered into multiple categories (except Patient’s Smile and Generosity of a Smile).
- Judges reserve the right to reallocate entries, if necessary.
- Closing date for all entries is 18 November 2011.
- All entries will be reviewed and then shortlisted. The shortlisted nominees will be notified mid January 2012.
- All information provided will be treated as confidential.
Cost of entering ~ Early bird offer
Pre-register before 14 October and be entitled to enter each category for £35+vat, thereafter the cost per entry is £50+vat. Entries deadline: 18 November 2011.

How to submit your entry
Each entry must have its own case report between 1000 and 1500 words. Please send in your report and clearly label each CD including all the high resolution photographs, completed entry form and payment to:
